By submitting this payment you do hereby acknowledge your application for active membership in NORTH DAKOTA FRATERNAL ORDER OF POLICE SOURIS VALLEY REGIONAL LODGE #7 has been approved and this payment is nonrefundable.
If you did not submit a written application with signed Oath and Obligation prior to receiving this link, STOP NOW and contact any Lodge 7 Executive Board Member.
If your membership should be revoked or discontinued for any cause, you do hereby agree to return to said Lodge your membership card and any other material bearing the F.O.P. insignia, such as auto emblem, lapel pin, etc.
A SEPARATE link for the Legal Defense Plan through the NDFOP State Unionly site will be sent to the email address you provided and will explain the application fees and premiums payment options. If these SEPARATE payments are not received, you will NOT be covered by Legal Defense. Retroactive coverage is NOT authorized.
