Membership Application

I hereby request membership in Choreographers Guild.

When eligibility is confirmed and accepted, I agree to be bound by the Bylaws of Choreographers Guild, the amendments thereto, and any policies and regulations now in effect or subsequently enacted by the Executive Board.

Once I am accepted as a member, I authorize Choreographers Guild to charge annual dues based on my self-estimated annual income from choreography based on the previous calendar year:

MEMBERSHIP ELIGIBILITY

To be eligible for membership in Choreographers Guild, you must have at least one choreography credit (including Assistant Choreographer, Associate Choreographer, Movement Coach, or related title) in a project covered by SAG-AFTRA, Actors Equity Association, AGMA, AGVA, or SDC, or submit a body of professional work for approval by the Executive Board or its designee.
